Shoulder rest/pad: build it up as high as you need. I suggest going to a foam/rubber shop and asking for off cuts, then use different densities/thickness to support the violin on your shoulder with correct posture. 2. Chin rest: consider building up the chin rest with cork (you will probably need to take your violin to a luthier/teacher to help you with this).
